Team Manager – Transition

Lead the way in establishing a new dynamic transition team for children and young people with complex disabilities in Southwark

The role

We need you to lead a new and dynamic transition team for young people with complex needs, aged 14 to 25. It’s a chance to make your mark as you implement self-directed support and personal budgets working in partnership with young people and their family carers. You’ll also manage a team of social workers and support staff.

The requirement

We’re looking for an experienced person with a passion and proven track record of implementing self-directed support and leading a team. A qualified and registered social worker, you’ll also have exceptional leadership and communication skills. A background in children’s and adult’s social work practice as well as supervising social workers will be essential.
